- Obv: Die crack from rim to top of Jefferson's head at 12:00; strong vertical die scratches in field to left of eye and lips; strong vertical die scratch behind head from R of LIBERTY to L of LIBERTY; strong vertical die scratches in field to right of queue.
- Rev: Horizontal die scratches in field to right of dome; strong strike doubling on E PLURIBUS UNUM.
